# Before your initial meeting: the questionnaire

## What our "Before we meet" questionnaire asks, what to have handy, and what we do with your answers.

"Before we meet" is a short questionnaire, linked in your booking confirmation. Your answers mean your adviser arrives knowing your situation, and the meeting is spent on what matters to you.

### What it asks

Each initial meeting has its own version. Most cover:

- You, your partner, and why you're seeking advice now
- Your super, savings, investments and property
- What comes in each year, and what you spend
- Plans such as travel, helping family or downsizing
- Your will, powers of attorney, accountant and solicitor
- For aged care or a village: the home or village you're looking at, and the family home

Family or an attorney can fill it in for a parent. Online, the aged care and village versions may be titled "Pre-Meeting Survey". For a Family Money Review, it's the Retirement Health Check version.

### Filling it in

Rough figures are fine, and you can skip anything you're unsure of. Please send it back about five days before your meeting. Rather do it by phone? Text us, and our client coordinator can go through it with you.

### What to have handy

- A recent statement for each super or pension account. This is the most useful one.
- Your latest Centrelink or DVA letter, if you receive a payment
- A statement for investments outside super
- For aged care: bank statements, the assessment letter, the home's fee schedule and the power of attorney, if you have them
- For a village: bank statements and the village contract, or its disclosure statement and fact sheet

### What we do with it

Our client services officers build your initial meeting pack from your answers. If you go ahead, they become the start of your fact find, so you're not asked twice.
