# The documents we need from you

## A checklist of the papers that help us get started, and what to do if you can't find something.

Send what you have, and we chase the rest. You only give us each document once. Upload them on [our secure upload page](https://form.jotform.com/243156689196067), or email them to admin@primeyears.com.au.

### For everyone

- Certified ID for everyone who will sign, including attorneys
- Your latest super and pension statements, for every account
- Three months of bank statements, for each account
- Statements for investments outside super, such as shares, managed funds and term deposits
- Your Centrelink income statement or latest letter, if you receive a payment or hold a card
- Your council rates notice
- Your will, and any enduring powers of attorney, financial and medical
- Your super death benefit nominations, if you have them
- Your accountant's and solicitor's names and contact details

### Retirement planning

- Your last two years' tax returns or notices of assessment. Your accountant can send them if you don't have copies.

### Aged care

- The resident agreement and fee schedule, if you have them. If not, we ask the home for them.
- The home's name, and a contact there if you have one
- Details of the family home: its value, any mortgage, and who lives there

### Retirement villages

- The village contract and the village's fee details, when you have them

### If something is hard to find

Tell us. Once you've signed your authorities, we can ask your funds, banks and providers for statements directly, so you don't have to chase anyone. If bank statements are a struggle, we can send you a secure link that helps.
